The applicant’s signature in the document to be notarized must be consistent with their signature in their passport and other government-issued valid IDs. Applicants must reflect their proper legal name in the document for notarization as it appears on their valid proof of identity.Applicants must present a valid proof of identity, such as a passport, Japanese government-issued Residence Card, or driver’s license.Document/s for notarization should be signed by the signatory in front of the consular processor on the day of filing. The Philippine Embassy does not notarize Judicial Affidavits following the rules set by the Philippine Supreme Court through its A.M.Apostilled documents no longer need authentication by the Philippine Embassy. Applicants who are unable to appear in person for consular notarization may have their document notarized by the Japanese Notary Public and apostilled by the Ministry of Foreign Affairs of Japan (Gaimusho).
